Pre-install customer document

Hi all,

I am putting together a pre-install document for my customers to fill out so that we can configure the system as much as possible prior to onsite install. Anyone have a doc that they want to share? Attached is my working document, feel free to modify\use.

One condition: if you modify, please make sure that you share it!

This is a good idea. Not only that you can make necessary adjustments to meet customer expectation. And also this saves time since customer will be able to write what is needed for his requirement.
